Factors influencing CMV seropositivity in stem cell transplant patients and donors

چکیده
Patient MV serological status remains an important factor for the outcome of allogeneic stem cell transplantation (SCT). CMV seronegative patients receiving a graft from a seronegative donor have lower transplant-related mortality. In a previous study using the EBMT registry, we found that CMV seropositive patients receiving grafts from seropositive unrelated donors had improved survival and reduced transplant-related mortality compared to those receiving grafts from CMV seronegative donors. Similar results were found in a single centre study by Ringdén et al. However, two other large studies have not been able to show any positive effect from using a CMV seropositive donor. Many different factors influence the risk of being CMV seropositive. The probability varies between different countries. One study suggests that the risk has been reduced in recent years. We used the registry of the European Group for Blood and Marrow Transplantation (EBMT) to look for trends over time regarding CMV serostatus and analyze the effects of age, gender and country.

Anti-CMV-IgG Positivity of Donors Is Beneficial for alloHSCT Recipients with Respect to the Better Short-Term Immunological Recovery and High Level of CD4+CD25high Lymphocytes
Hematopoietic stem cell transplantation from anti-cytomegalovirus immunoglobulin G (anti-CMV-IgG) positive donors facilitated immunological recovery post-transplant, which may indicate that chronic CMV infection has an effect on the immune system. This can be seen in the recipients after reconstitution with donor lymphocytes. We evaluated the composition of lymphocytes at hematologic recovery i...
متن کاملIncidence and risk factors for cytomegalovirus in kidney transplant patients in Babol, northern Iran
Background: Cytomegalovirus (CMV) disease is an important cause of death and possibly transplant rejection in kidney transplant (KT) patients. This study was conducted to investigate the incidence and risk factors of CMV disease in kidney transplant patients. Methods: All end-stage renal disease (ESRD) patients who underwent kidney transplantation during 1998-2014 and their donors were assessed...
متن کاملImproving Cytomegalovirus-Specific T Cell Reconstitution after Haploidentical Stem Cell Transplantation
Cytomegalovirus (CMV) infection and delayed immune reconstitution (IR) remain serious obstacles for successful haploidentical stem cell transplantation (haplo-SCT). CMV-specific IR varied according to whether patients received manipulated/unmanipulated grafts or myeloablative/reduced intensity conditioning. CMV infection commonly occurs following impaired IR of T cell and its subsets. Here, we ...
متن کاملEditorial commentary: the complexity of latent cytomegalovirus infection in stem cell donors.
In solid organ transplant and in stem cell transplant, the impact of the cytomegalovirus (CMV) serologic status of both the donor and recipient have been the subject of intense scrutiny [1, 2]. In both types of transplants, it has been clear that CMV status can play a significant role in the outcome. In solid organ transplant, CMV donor seropositivity has been well established to lead to an inc...
متن کاملHematopoietic stem cell transplant therapy, clinical trials, complications, and quality of life for patients with Sickle cell anemia: Clinical potential and future perspectives
Background: Sickle cell anemia (SCA) is an inherited monogenic disorder. The clinical symptoms of SCA are protean, including vaso-occlusion, hemolysis, early stroke, leg ulcers, multi-organ failure, and increased risk of premature death. Hematopoietic stem cell transplantation is the only treatment identified to reduce SCA-related organ damage. Unfortunately, graft rejection is a significant im...
